State service "Acceptance of documents and enrollment in the organization of education, regardless of departmental subordination for training in general educational programs of primary, basic secondary, general secondary education"
To receive the public service "acceptance of documents and enrollment in the organization of education, regardless of departmental subordination for training in general educational programs of primary, basic secondary, general secondary education" acceptance of documents and issuing the results of the provision of public services are carried out through the web portal of the "electronic government" (hereinafter referred to as the portal) and on paper through organizations of primary, basic secondary education (hereinafter referred to as the service provider).
To receive the public service, parents or other legal representatives of the child (hereinafter referred to as the service recipient) provide the service provider in accordance with the list of basic requirements for the provision of public services approved in accordance with Appendix 1 to these Rules (hereinafter referred to as the List).
List of documents necessary for the provision of public services:
-c servant (paper):
1) the application of parents or other legal representatives in accordance with the form to the list of basic requirements for the provision of public services: “Acceptance of documents and enrollment in the organization of education, regardless of departmental subordination for training according to general educational programs of primary, basic secondary, general secondary education”;
2) an identity document (the original is required for identification, which is returned to the service recipient) or an electronic document received from the digital documents service;
3) Medical certificates of form No. 065/U “Map of preventive vaccinations” and forms No. 052-2/U “Child Health Passport”, approved by order of the acting Minister of Health of the Republic of Kazakhstan dated October 30, 2020, ФЗM-175/2020 “On approval of forms of accounting documentation in the field of healthcare, as well as instructions for filling them” (registered in the register of state registration of regulatory legal acts under No. 21579);
4) photos of a child in size 3x4 cm in an amount of 2 pieces.-on the portal:
1) the application of parents or other legal representatives in accordance with the form to the list of basic requirements for the provision of public services: “Acceptance of documents and enrollment in the organization of education, regardless of departmental subordination for training according to general educational programs of primary, basic secondary, general secondary education”;
2) Medical certificates of form No. 065/U “Map of preventive vaccinations” and forms No. 052-2/u “Child Health Passport”, approved by order of the acting Minister of Health of the Republic of Kazakhstan dated October 30, 2020, ФЗM-175/2020 "On approval of forms of accounting documentation in the field of healthcare, as well as instructions for filling them" (registered in the register of state-owned state registration of regulatory legal acts under No. 21579);
3) a digital photograph of a child in size 3x4 cm.

The application on the portals is signed with the electronic digital signature of the parents or other legal representatives. Dear parents, please note that the gymnasium school has gymnasium classes and general education classes. Due to the changed rules of admission to the 1st gymnasium class, we do not have the opportunity to determine the professional assessment of the level of knowledge and psychological readiness of children to study in the gymnasium class. When making a choice in favor of teaching your child in a gymnasium class, you need to understand and objectively assess the child's readiness for the academic load, which is complemented by a gymnasium component. Your child is a future first grader. His academic success is the interaction of a child, a teacher, and a parent.
